Smegma how do you get rid of this , im 44 went to my GP last year he gave me canaston cream with no effect?

first noticed it approx 2 weeks after with my exwife.who had split up with her partner 2 months previous, i have never been unclean in my area,does my ex need to use caneston or is there something else i should be using.
